Two networks included: Delta Dental Preferred Provider Organization (highest savings) and Delta Dental Premier (still in-network with savings, but less than Preferred Provider Organization)
100% coverage of all in-network preventative and diagnostic expenses, including checkups and cleanings twice per calendar year, dental x-rays, and fluoride treatments for children under age nineteen
$25 deductible ($75 maximum deductible per family); does not apply to preventative services
80% coverage of basic dental work, including fillings, oral surgery, endodontics, and anesthesia
50% coverage of more extensive work such as implants, crowns, bridges, and dentures
50% coverage of orthodontia (lifetime maximum of $1,500 per person)
$1,500 maximum coverage per year, per person which can be carried over to the next calendar year for a maximum of $3,000 per year, as long as you were enrolled with Bethany Benefit Service for the entire previous year and filed at least one claim the previous year
Increased number of cleanings per year for individuals who have diabetes, are pregnant, have periodontal disease, have kidney failure, have high-risk cardiac conditions or suppressed immune systems, or are undergoing cancer-related radiation or chemotherapy (download the Enhanced Benefits Program below for more details)
